I am emailing to send you information about the dental screenings for your school on Friday Oct. 13th. On that day, I will need the following things;

A room or space to screen in.- art room
An adult size table and 2 chairs.
A trash can with a liner.
A volunteer to record for me.- Diane Fulbright (sending home background check)
Class rosters from your data manager. I can work with your data manager on this.
A schedule… to help with this see the memo in the attachment. Save the memo to your desk top, add in teacher names next to times, change the sentence that states where the screening will be held and email out to teachers.
I use a 15 minute time slot per class for the screenings. This works really well if the teachers are on time, with children in alphabetical order with their health cards in their hands. After screening the children I will be filling out results on parent forms and the health cards. Each child will have a form that goes home to the parent.

Beckie Boulton, the Public Health Dental Hygienist will conduct dental screenings on Friday October 13, 2017.
Screening will be done in the art room. Health cards will need to be pulled from the cumulative folders. Students should be placed in alphabetical order and arrive at the screening site with health cards in their hands. The screening schedule is very tight; therefore, it is very important for each class to arrive on time.
If your class is scheduled after snack or lunch, please give the students a chance to rinse their mouths before you bring them to be screened. Thank you for your help.
